I'm Sivaprasad "Shiva" Parnam, PT, DPT, a physical therapist and the founder and CEO of Shiva Physical Therapy in Apex, North Carolina. My cash-based practice is built around Regenerative Physical Therapy, a non-invasive, whole-system approach to musculoskeletal rehabilitation that works at three connected levels: the tissue, the nervous system, and the mind.

I've worked with shockwave therapy for 12 years and with Extracorporeal Magnetotransduction Therapy (EMTT) for the past 3. In my clinic, radial and focused shockwave, EMTT, Class IV laser, NESA non-invasive neuromodulation, StimPod peripheral nerve stimulation, and other regenerative modalities are combined with manual therapy, progressive loading exercises, and pain neuroscience education. I especially enjoy treating people with chronic musculoskeletal conditions that haven't responded to standard care, including frozen shoulder, tennis elbow, plantar fasciitis, and persistent low back and neck pain.

My work centers on helping Regenerative Physical Therapy become a recognized, evidence-based clinical discipline. I created The S.H.I.V.A. Method®, a 5-phase regenerative framework®, and I'm currently writing a clinical textbook on Regenerative Physical Therapy. At the APTA North Carolina 2026 Annual Conference, I'll be presenting on shockwave therapy in physical therapy, covering mechanotransduction science, clinical application, and scope of practice.

I hold a Doctor of Physical Therapy degree and have more than two decades of experience in clinical practice and rehabilitation leadership. My postgraduate training includes Manual Medicine training through the Michigan State University College of Osteopathic Medicine and a Diploma in Osteopractic from the American Academy of Manipulative Therapy. I also hold certifications in Spinal Manipulation, Dry Needling, Musculoskeletal Ultrasound, Vestibular Rehabilitation, and Stroke Rehabilitation, along with the Expert in Non-Invasive Neuromodulation credential from the University of Las Palmas de Gran Canaria. I'm also a licensed Nursing Home Administrator in North Carolina.
